- Have not been convicted of a felony, significant misdemeanor, or multiple misdemeanors
- Applicants must also be at least 15 years old at the time of filing, unless they are currently in removal proceedings or have a final removal/deportation order.

- Passport, birth certificate, government-issued photo ID
- Evidence of Arrival in the U.S.
- I-94 travel record, visa entry stamp, or school or medical records showing presence in the U.S.

- Proof of Physical Presence on June 15, 2012
- Dated receipts, school records, or employment records showing presence on that date
- Educational or Military Records
